Contact Kaiser for medical ID cards, in-network providers, claims, or coverage questions.
Dental, vision, life, and disability coverage questions should go directly to MetLife.
Flexible Spending Account (FSA) and Health Savings Account (HSA) administration and debit cards.

Confidential counseling, legal and financial consultations, and work/life resources are delivered through Canopy’s Employee Assistance Program.
Flexible Spending Accounts and Health Savings Accounts are administered by WEX Health and managed through Xenium. Use their portal to submit claims, view balances, and access debit card info.
💳 Access WEX Benefits PortalCritical illness, accident insurance, hospital indemnity, and other optional programs through Aflac. Enroll directly with Aflac (not through Prism).
📄 Aflac Voluntary Benefits OverviewYou can change elections during open enrollment or within 30 days of a qualifying life event (marriage, birth, adoption, loss of other coverage, etc.).
Members on the Kaiser HMO will most likely need to select a new Kaiser primary care doctor. Dual Choice members may be able to keep their current provider by confirming participation through the First Choice Health Provider Search tool.
Most carriers provide digital ID cards in their mobile apps and member portals. Physical cards may also be mailed to your home.
Start with the carrier’s member services. If you need additional help, contact the broker helpdesk or HR.
Because this is an active enrollment, coverage does not roll over. Missing the deadline means you’ll be without elections until next year unless you have a qualifying life event (marriage, birth, loss of coverage, etc.) within 30 days.
